Spanish equivalent of E111

Can anyone tell me the name of the Spanish equivalent of E111 and how to obtain one please. (I do have Spanish health cover)

Ive tried search without success.

formulario E-111 en cualquier oficina de la Seguridad Social. Para ello necesitarán acudir con el Documento Nacional de Identidad y la cartilla de la Seguridad Social.

The E111 form is no longer issued as it has been replaced by the new European Health Card.

thats true, i have one :) i got it from the uk tho, i applied for a e111 to go to cyprus with, got that from the post office. i guess they send them out to the addresses that they have, i´m not sure. it just came in the post :)
Alhaurin el Grande since 99, working at the airport since 2011.
