I am trying to find this to download online but not having any success - does anyone know the link by any chance - tried TGSS and it does not come up, its the solicitud I need.

I don't think that you can download an E112.
This was a UK form issued until replaced by the S2 form.
The S2 form is only produced by Newcastle to qualifying people upon request.
