Yes, at the time you exchanged, licences for over 70s were only valid for 2 years. Between then and your renewal they changed to to 5 years.

The one I used in Velez-Malaga needed no appointment, just me, my current permiso and 40€! It was the same for my other half to get the medical before changing UK to Spanish - more convenient to get this done before going to Trafico in Malaga, where the touts outside will pester you to go to "their" medical centre.
I used to cough to disguise a [email protected] I f@rt to disguise a cough.
