IS EX-18 the correct form?

I want a medical card for my retired mother who is living here in Seville with me. She receives her UK pension. In order to get the medical card, I need Residencia for her. The office in Plaza de España where I apply for the Residencia has given me an EX-18 form. Is that the right form for a retired person living in Spain? Thanks in advance for your help.

Yes. You will also need to prove adequate income and you will need form S1 from DWP in UK to show that she will be covered for healthcare.
