EX-18 Completed form example

This might be a help to anyone trying to complete the EX-18 form to sign on the Foreigners Register which we are supposed to do if we live in Spain more than 3 months.
There have been other threads about it, and indeed I have also contributed, but I think to actually see what a completed form looks like might be helpful. The form can be completed online and then printed out which is what I did and I used the format that was successful for us. Note that piso is not "floor" but on the form actually means "apartment" so if you are in a block of apartments as we are, you enter the building number and then the apartment no.

That form download link doesn't work on any of my devices - it used to so it may be a temporary problem.

If anyone has the same trouble with it, this one does work

Err......me again! The form we downloaded from Sid's link is different at the bottom from Fyfin's example. Do we just put a cross against the option 'National de Estado miembro de la Union Europea"?

I have managed to get the official form up from Fyfins link using IE (which I hate!!!) rather than Chrome.

The link I gave seems to be an older version of the form. The one in Fyfins link is the latest and I agree that you tick the third option under residencia temporal - No Activo etc which is what Fyfin suggested.

Link doesn't work in Firefox either. :thumbup:
But using the Firefox Addon , IETab, and clicking Open Link in IETab it opens as if it had been opened in IE. :thumbup:
Doesn't Chrome have a similar Addon?

What about the 2 at the bottom, Sid? we don't tick the first one, do we, as we don't have private medical insurance?

At the moment, we are struggling as our adobe reader seems to be out of date, we are trying to upload the latest version :( we can't get the form up.

Irene download and install FoxitReader from http://www.foxitsoftware.com/Secure_PDF_Reader/
It's free and better than Adobe reader.

Agreed - that's what I use but the link still won't open!

It seems to be demanding Adobe, which I also have but that doesn't seem to work either.

I will see if Chrome has a suitable addon.

Leave it unticked as you should have your medical cards by then.

If in doubt the people in the office will advise you.

Works ok in Palemoon Browser.............with Adobe reader

It has but it still cannot handle that link either on the PC or the iMac.Same error message on both.

Adobe Reader..........uncheck the McAfee........"crap"

Yes, we unchecked McAfee (spit!) Enrique but computer say 'No' - we get to 72% uploaded then it grinds to a halt :( Gerry, your reader uploaded beautifully but it says that particular file is 'incompatible'. So - after a short case of the screaming habdabs, we have decided to take our many, many copies of this, that and the other in to the police station tomorrow and act dumb (which won't be difficult) when we get told the EX18 is out of date. Presumably they WILL be able to supply us with the correct form :think: and we will no doubt have at least one more trip. Thanks so much for trying to help, guys - you are all lovely!

If you download the forms from the Ministry of Employment/Social Security website, as per Enrique's link, you then complete them on your computer before printing them.
You don't have to print them and then fill them in with a pen. :thumbup:
