We seem to have lost my wife's card, although I might have left it in the Pharmacia, won't know until tomorrow.
How does one go about getting a replacement and what documentation is required?
We have the numbers off it.

I think you can just ask at your medical center.

Yes, just go to the medical centre where you are registered.
They will order a new one for you.

Or, as I have discovered this morning, the Pharmacia. Needs NIE number and NUSA, name and address as on record.

I seem to recall it's your Andalucia Health number - hubby had one years ago (not that he was resident, it just made life easier for the Medical centre !)

I did a reply to Gerry a while ago but did not press submit, it's an age thing.
In the Pharmacy she wanted NIE, NUHSA (ie the ANXXXXXXXX) number, name and address as known by the health service.
Bingo, she provides a piece of paper with your numbers and acknowledgement that you have requested a replacement card.
If you were to go to the doctor and get a new scrip he puts it on the computer in the normal way, you show your paper at the pharmacia, they enter your number manually and you get your meds.
Same goes for repeats.
The only bit I am taking with a pinch of salt is she said new card in a week.

I have taken photos of my ID cards back and front, other documents and the dog's. Handy to have all the data on the phone.
